All
applicants must submit a completed application form and a nonrefundable
$50 application fee. A $1000 deposit, applied toward tuition, is
due upon notification of acceptance, with the balance due by April
15th. The deposit is refundable only if cancellation is made prior
to April 1st. After April 1st the deposit is non-refundable. Tuition,
less the $1000 deposit, and activity fees are refundable prior to
April 15th. No money is refunded after April 15th. Any tuition or
fees not paid in full by April 15th will result in a loss of enrollment,
unless prior payment arrangements have been made. Any enrollments
received after April 15th, are due in full upon notification of
acceptance.

directly to Perry-Mansfield
High School & College students will not be
considered until the above is completed. Upon notification of
acceptance, a $1000 deposit is due to reserve space.
High School & College students will be informed
of both scholarship awards and acceptance by March 15th.
Upon acceptance and receipt of your $1000 deposit,
we will send you an information packet containing all necessary
forms and the Student Handbook. Please fill out all forms and
return them to Perry-Mansfield. Be certain to sign and return
the Student/Parent contract in the packet.
Enrollment Video Criteria:
High School & College students wishing to enroll must send
an enrollment video. Please use the criteria listed below for
your video. Students cannot use production videos. Videos
must be in VHS or DVD format and must show the full body for the
purpose of evaluating stage presence. Students are enrolled on
a first-come, first-served basis. Do not delay in sending your
enrollment information.
Notes:
High School & College applicants who audition for a scholarship
by video do not need to send a separate enrollment
video.
Returning students from the previous summer High School &
College program do not have to submit a videotape for enrollment
with the exception of Level 1 dance students.

in one of your dance classes, and a recently taped solo no
more than two minutes in length.

| Theater: |
Two short contrasting monologues, contemporary
or classical, limited to two minutes each.
 |
| Musical Theater: |
Two contrasting songs from the musical theater
literature. For students interested in both the theater and
musical theater programs, send a video consisting of one song
and one monologue. |

Day Students: Day students may register for one
to five classes, but must register for a minimum of two classes
to participate in the theater, muscial theater or dance productions,
and must then attend evening rehearsals. Day student tuition includes
lunch.
